| About the Care Partner Program |
| The Care Partners Program at West Park Hospital is part of our Planetree philosophy of patient-centered care.
Our goal is to involve a family member or friend to be a part of the health care team, thus enhancing the patient’s experience in and out of the hospital.
The patient is given the chance to choose a Care Partner to assist them while in the hospital and later at home.
As a patient, when you first arrive you will be asked to identify who will be helping you at home. This will assist the staff in helping you make a decision about a Care Partner.

| What Is a Care Partner? |
| A Care Partner is someone of the patient’s choosing who has the time and is willing and able to learn how to help their family member or friend recover during their stay and after they leave the hospital.
As an important member of the health care team, Care Partners can be vital to the overall health and healing process of the patient. As a team member, the Care Partner is empowered to learn and to participate in the recovery of their loved one.

| Who Makes a Good Care Partner? |
The best Care Partners are family members or friends of the patient who have access to them at home and are able to fulfill the responsibilities of being a Care Partner.

| What are a Care Partner's Responsibilities? |
Communication
- Spokesperson to family and friends regarding the patient’s progress
- Write down questions for physicians
- Ask questions of health care team members
- Participate in the discharge planning process and contribute to a timely discharge
- Keep track of personal items
- Help compile, complete and update patient’s Personal Information Book
- Creating and maintaining a CarePage for patient
Education
- Learn about home care
- Medications
- Dressing changes & skin care
- Activity & mobility (walking)
- Restrictions & precautions
- Equipment (walkers, wheel chairs, slings, etc.)
Physical
- Incentive spirometry
- Assist with menu selections for meals and snacks
- Comfort measures — cool washcloth, warm blanket, etc.
- Keep track of fluid intake & urine output
- Activity — work with therapy on mobility
Emotional
- Visiting, encouragement, positive coaching
- Assist with phone calls
- Coordinate rest & visitation hours
Spiritual
- Prayer
- Request a chaplain visit or set up a visit with the patient’s spiritual advisor
