These pre-paid, self-referred tests can screen for anemia, cancer, diabetes and cardiac risk.
You can request these health-screening tests without a doctor’s order through West Park Health Check. This service offers high-quality, affordable health screening. Testing is confidential and can provide you an important tool for wellness assessment and early detection of some disease. You order the test; you receive the results.
Use the list below to determine which tests you need, print and fill out the Consent and Liability forms and drop in between 7 a.m. and 3:30 p.m. Monday-Thursday or 7 a.m.-12:30 p.m. Friday. It's that simple.
For accurate results, this type of health screening requires fasting
(nothing by mouth except for water) for 10 hours to 12 hours beforehand
(Diabetics should not fast for more than eight hours,
and all participants should continue taking medications as prescribed)
